There are five documents you need to complete your reservations and be ready for your charter:
1. Charter Agreement
2. Skipper Resume Form
3. Manifest
4. Taxi Transfer Form
5. Provisioning and Beverage Form
All documents are provided for download using Adobe Acrobat PDF format. You must have Adobe Acrobat Reader installed in order to view these documents. If you do not have Adobe Acrobat, please click here to download it for free. To download, simply click on the files below and a new browser window will open with your document already loaded.
Once the document is open in your browser, you may either fill it out on-line and then print it, or print it first and fill it out by hand. Unless you have the full (paid) version of Adobe Acrobat, you will not be able to re-save the form once it is completed, so make sure that you print the form before you close it.
